What is Care Coordination?
Solo Aging Solutions, LLC (SAS) provides care management and coordination services to our clients as they navigate their healthcare and work to maintain their quality of life. We assist our clients with formulating a plan tailored to their specific needs and wishes for aging and future care. Ongoing coordination of medical care, living arrangements, and advance planning is provided to alleviate stress and provide direction as challenges arise.
Advance Care Plan
Your Local Care Coordinator (LCC) will develop a comprehensive advance care plan and conduct ongoing monitoring.
Local Care Coordination
Your LCC will help identify and arrange for proper care, whether in-home or placement in a care community. LCCs also coordinate with medical providers, advocate on your behalf, and refer local community resources.
Continuing Support
Your LCC will provide local oversight and support for long-distance caregivers. In the event of hospitalization, your LCC will support with discharge planning and mapping out a care plan post-hospitalization. LCCs are prepared to coordinate home health or hospice services if the situation calls for it.
